A: The Server Cleanup Wizard does not remove updates from the database for valid updates, so in this case it's not going to be much help.
The issue here is not what you need to remove, but most likely what you've synchronized to begin with. My guess would be that you enabled all classifications (including Drivers) -- and it's the "Drivers" update classification that loads the extra 15,000 updates in your database.
For that scenario, the only fix is to uninstall the WSUS server, delete the database, and reinstall WSUS with a new database and NOT select the "Drivers" update classification for synchronization.
